To better understand and meet the needs of our students and planet, SJSU is conducting a sustainability survey. The assessment focuses on awareness, attitudes, and culture. The information you provide will be used to make recommendations on programming and resources that will support sustainability teaching and research on campus. Thank you for taking five minutes out of your day to participate. 

You are being asked to participate in a research study investigating your sustainability opinions and awareness The survey is completely voluntary and confidential. It will take about 5 minutes to complete. Potential risks from participating in this study are no greater than those encountered in everyday life. You will not receive any direct benefit from this study. You will not be compensated for your participation. No service of any kind, to which you are otherwise entitled, will be lost or jeopardized if you choose to “not participate” in the study. Your consent is being given voluntarily. You may refuse to participate in the entire study or in any part of the study. If you decide to participate in the study, you are free to withdraw at any time without any negative effect on your relations with San Jose State University or with any other participating institutions or agencies.

Although the results of this study may be published, no information that could identify you will be included.
