Please be informed that Narcan is available to SJSU students, staff and faculty only.
Students:
Before requesting Narcan, please complete a brief training video. This video contains information in recognizing signs and symptoms of opioid overdose; how to administer Narcan, and a pre/post-test. Your test results will only be used to help Student Wellness Center measure baseline knowledge of the topic and evaluate training effectiveness. They will NOT impact your eligibility to request for Narcan.
Ensure that all sections of the form are completed prior to submission.
Staff/Faculty:

*Note to returning students/staff/faculty*
To pick up Narcan, the brief training video must be completed once per academic year. If you have not completed the training during the current academic year, please continue to the next page. If you have completed the training during the current academic year, you are no longer required to submit this form and can proceed to pick up Narcan at the front desk of the Wellness Lounge (Student Wellness Center) during business hours.
