To help us determine if you meet the qualifications to take courses in the Business Analytics Certificate program, please provide all of the information below.  This form must be submitted at least one week prior to the start of the class; however, if you were educated outside of the United States, you will also need to apply through Cal State Apply (https://www2.calstate.edu/apply) if this pre-screening is approved- this could take several weeks.  You and the course instructor will be notified of the pre-screening results.

Please note: This program is 100% online and you can complete it from wherever you are located.  San Jose State University will not issue an I-20 to secure an F-1 visa to enter the United States for this program.
