Please note that you must use a computer (not a mobile device or tablet) to complete this application. You will be required to upload several documents. Please have your Statement of Purpose and Resume prepared and ready to upload during this application process.

Thank you for your interest in the San José State University MPH Program. Please complete this no-charge/no-fee application survey by the designated deadline.

Campus and Online Format:

Applications for Fall 2025 (FA25) will be accepted starting October 1, 2024. The MPH application deadline dates are:

April 1, 2025 for International Rolling Admission deadline
June 1, 2025* for Domestic Rolling Admission deadline [*We highly recommend to apply before May 1, 2025 due to enrollment capacity. If the program meets capacity before June 1, outstanding applications may be placed on a waitlist].

Please be advised SJSU has separate application deadlines:

International Applications: April 1, 2025 (document deadline: April 20, 2025)
Domestic Applications due: June 1, 2025** (document deadline: June 20, 2025). **We highly recommend to apply before May 1, 2025 due to enrollment capacity. If the program meets capacity before June 1, outstanding applications may be placed on a waitlist.

- Campus Week is held on SJSU's main campus during the first full week of August, but in-person sessions are held for 3 full days.

What led you to public health.
How in your past and/or current work/volunteering you have engaged with diverse cultures and/or communities, using specific examples.
A discussion of any admission criteria that you do not meet, such as lower GPA or lack or work experience, and plans for addressing those areas if admitted to the program.
Why you want to pursue a masters degree in public health (particularly in our program, an MPH in Community Health Education).
How receiving an MPH from our program will help you achieve your professional goals.
What strengths/qualities you will bring to the SJSU MPH program and the SJSU Spartan community.
