Itemized Statement Request
This form should be completed by current or past clients of the Student Wellness Center at San José State University who are interested in receiving an itemized statement.
Students can always see a listing of charges in the
SWC Patient Portal in the Account Summary Tab.
Itemized statements typically take 3-5 business days to process. Processing will be delayed if all sections are not completed.
