To help us determine if you meet the qualifications to take courses in the Advanced Global Leadership Certificate program, please provide all of the information below.  This form must be submitted at least two weeks prior to the start of the class; however, if you were educated outside of the United States, you will also need to apply through Cal State Apply (https://www2.calstate.edu/apply) if this pre-screening is approved- this could take several weeks.  You will be notified of the pre-screening results.
